5. Platinum Plus:

Terms & Conditions:

You may choose from one of our six types of memberships. Memberships must be used under our Registration Policy only. Which means You must register for Day Class at least 1 hours before hand. Your membership will become effective the date you signed up for it. Your Membership will expire the following six weeks.  Memberships are non- refundable. You Are responsible for utilizing your Membership during the course that it is active. Members can renew their membership for $20 off before their active Membership Expires. If you choose not to renew your membership you will be removed from from all Membership Services. If you wish to obtain a Membership after your Original membership has expired you will be charged a $15 activation fee. By obtaining a platinum Dance Studio Membership you Agree to the following: I realize that my membership can be terminated by boisterous behavior, alcohol intoxication or the use of illegal drugs, Not Complying with the Register Policy. Disrespecting staff., Or Clients of platinum Dance Studio. WAIVER: In consideration of the benefits derived from the Platinum Dance Studio, I (we) do hereby agree to indemnify and hold harmless, release and discharge the sponsoring organization of Platinum Dance Studio, of Philadelphia , PA, it’s agents, members, instructors, officers or employees, from and against all liability, claims, suits, damages, losses and expenses, including attorney fees, threatened or incurred, including any and all consequential damages claims which I (we) may be entitled to recover from said injury or property damage claim and arising from the persons participation in Platinum Dance Studio, activities related to dance or by reason of any injury or damage to said to any persons or property occurring during said participation, or from any cause whatsoever. I (we) fully realize that activities at Platinum Dance Studio, can be dangerous and could result in serious injury or possible death and freely assume that risk. I (we) give no fault of Platinum Dance Studio.

I have read and agree to abide by the agreement, policies and procedures of Platinum Dance Studio. I Have not had any drugs or alcohol. I do not have any serious medical conditions, such as heart, lung spine, or brain. I understand that shoes are an optional part of the lesson. It is my free will to wear shoes during the lessons . I will experience Pole Dance, Yoga, Salsa Fitness, & Sensual dance at my own risk..
